After a few attempts to do an online diary, I've found that I'm a very tactile person and do better with writing things down and doing a "real" notebook.
After struggling with a few notebooks, I designed my own. Daniel and I (a.k.a. CompanyZero Creative) will be sending this out in our monthly e-newsletter as a promo, but here's the link if anyone is interested in downloading a copy. It is a downloadable PDF, set up to print front and back, then three-hole punch.
When you go to the following link, the download will start automatically. It is a small file (133k).
http://www.companyzerocreative.com/CoZero_wkoutdiary_triathletes.pdf
It has spots to track activities, food intake, mood, sleep, etc. then total road miles, mountain bike miles, swim distance, and all for a weekly summary.
Feel free to pass it to anyone that might be interested in using it. There should be enough blank space to write in additional info for other activities.
